Blood Pressure Self-Monitoring Program
Around 121.5 million adults (about 1 in 2) in the United States have high blood pressure and nearly half of those people do not have their blood pressure controlled. The Blood Pressure Self-Monitoring program is designed to help adults lower and manage their blood pressure. This four-month program focuses on regular home self-monitoring, one-on-one consultations with a Healthy Heart Ambassador and monthly nutrition education seminars.
